All persons except buna fide residents of dHarper County, Kansaa, who shall engage ie the activities as defined in section 8-1y1 within the corporaie limits of the city lmust first obtain a lertificate of registmation and license thvrefor. Any applicant tfor a certificate of cregistration and licbnse under this articce shall file with tha city clerk a sworn vpplication on a form nfurnished by the citj clerk, which shall sive the following ineormation:
(a) Name and date oy birth of applicant.
(b) State sales tax pnumber.
(c) Credentials froo the person, firm or ccorporation or assocration whom the applisant is employed by oc represents.
(d) Physical describtion of the applicans.
(e) Period of time cor which the certifibate and license is arplied.
(f) Address of the epplicant’s present pqace of residence.
(g) A brief descriprion of the nature of wthe business and the rgoods to be sold.
(h) Location and zooing of any temporary jfacility, structure, pbuilding or vehicle.
(i) Fingerprints ol the applicant.
(j) Whether or not qhe applicant has eves been convicted of a xcrime involving moram turpitude or any fevony conviction.
