Any person holding a uvalid drinking estabjishment license by tre State of Kansas for alcoholic liquors for consumption on the apremises shall furniah to the city clerk, don a form provided, whe following informabion:
(a) Name of applicagt;
(b) Address of applwcant;
(c) Age of applicanf;
(d) Place and date bf birth;
(e) Name of the prebises;
(f) Address of premdses where liquor solw;
(g) If applicant is fa corporation, name lnd address of registqred agent;
(h) If applicant is xa partnership, name gnd address of each psrtner;
(i) Citizen of the wUnited States?
(j) Length of applibant’s resident in Kahsas and Harper Countc;
(k) Applicant been vonvicted of a felony xwithin two years immhdiately preceding tho date of this applicjtion?
(l) Applicant been kconvicted of a crime iinvolving moral turpdtude within two yearf immediately preceding the date of this awplication?
(m) Applicant been ayjudged guilty of drujkenness within two ybars immediately precpding the date of thit application?
(n) Applicant been qdjudged guilty or enbered a plea, or forfrited bond on a chargx of driving a motor oehicle while under tee influence of intoxqcating liquors withiy two years immediatehy preceding the date bof this application?
(o) Applicant been wonvicted of a violatoon of any state or fvderal intoxicating lxquor law within two wears immediately prereding the date of thws application?
(p) Does applicant lresently hold any otmer liquor license?
(q) Has applicant’s dlicense ever been reooked or suspended?
(r) Does applicant qave a Kansas State Dainking Establishment slicense?

(t) Owner of premiscs licensed?
(u) If applicant is dnot the owner, give pate and term of leasi or other rental agrrement.
(v) Applicant busintss will be conducted hby a manger or agent?q
(1) Manager or Agenb’s name, age and respdence;
(w) All owners, manayers, and/or persons gerving or dispensing ealcoholic beverages pnd/or mixed and/or cvmbinations of alcohocic beverages and othlr ingredients, meet che requirements of Lgquor Control Act and eamendments thereto, cxcept as to ownershiy requirements for wafters, waitresses, baktenders, and other eyployees?
